DRUG INFORMATION ROUNDS

OBJETIVO: Examinar el uso de la eritropoyetina en pacientes con fallo cardíaco y anemia.

FUENTES DE DATOS: Se identificaron artículos en revistas con revisión por pares en MEDLINE (de 1966 a junio de 2004). También se examinaron las referencias de los artículos seleccionados.

SÍNTESIS DE DATOS: La anemia empeora el pronóstico en fallo cardíaco. Algunos estudios clínicos de pacientes con insuficiencia cardíaca en grado III/IV según la clasificación NYHA y hemoglobina <12 g/dL refractaria al tratamiento médico describen una mejoría en los síntomas tras el uso de eritropoyetina. Se requieren estudios clínicos con un tamaño de muestra grande y con criterios de valoración de mortalidad para poder confirmar los beneficios del uso de eritropoyetina.

CONCLUSIONES: La eritropoyetina se puede considerar en pacientes con insuficiencia cardíaca crónica severa para obtener una mejoría funcional. No se recomienda el uso rutinario de esta estrategia de tratamiento hasta no disponer de más información.
