OBJETIVO: Informar el caso de un paciente que sufrió rabdomiólisis y fallo renal agudo asociado con monoterapia de gemfibrozil para hiperlipidemia.

RESUMEN DEL CASO: Un hombre de 30 años con hipertensión, diabetes mellitus tipo I y e hiperlipidemia fue hospitalizado debido a mialgias, nausea, vómito. Los síntomas comenzaron 4 días antes de la hospitalización, cuando el paciente comenzó un trabajo como operador de martillo mecánico. Previo a su hospitalización, sus medicamentos eran lisinopril, aspirina, insulina, y gemfibrozil. Sus laboratorios revelaron una elevación marcada de kinasa de creatina y creatinina lo cual es consistente con rabdomiólisis y fallo renal agudo.

DISCUSIÓN: Este es el único informe hasta la fecha de un paciente con una creatinina normal documentada el cual desarrolla rabdomiólisis con fallo renal agudo asociado con el uso de gemfibrozil como monoterapia para hiperlipidemia. El haberse ejercitado vigorosamente, hipovolemia, y el uso de lisinopril puede haber contribuido a la severidad de la condición. Una evaluación de causalidad objetiva reveló que el efecto adverso a droga causado por gemfibrozil es posible.

CONCLUSIONES: La monoterapia para hiperlipidemia puede predisponer para rabdomiólisis con fallo renal agudo. Los pacientes usando gemfibrozil deben ser prevenidos de que el ejercicio vigoroso, la deshidratación y la necesidad de una evaluación de sus mialgia sin demora.
