OBJETIVO: Reportar un caso de una interacción entre jenjibre y fenprocumona resultando en epistaxis y en una elevación del valor internacionalizado normalizado (INR por sus siglas en inglés).

RESUMEN DEL CASO: Una mujer europea de 76 años de edad desarrolló una elevación de INR de hasta 10 y epistaxis como resultado de una interacción entre fenprocumona y jenjibre.

DISCUSIÓN: Se han realizado un número de investigaciones que han producido opiniones conflictivas sobre el efecto del jenjibre en la homeostasia y específicamente sobre la inhibición de plaquetas. Aún así, en base a estas investigaciones, se han emitido recomendaciones para abstenerse de la ingestión de jenjibre y otros productos herbarios, como el ajo o el ginko, en situaciones donde la hemorragia puede ser crítica. Una evaluación de causalidad objetiva reveló que el evento adverso entre fenprocumona y jenjibre fue posible.

CONCLUSIONES: Este es el primer reporte de caso que podría apoyar una interacción entre un anticoagulante oral y jenjibre, junto con un breve repaso de la literatura sobre jenjibre y homeostasia. Debido a que la observación de esta interacción fue fortuita, este caso también señala la importancia del auto-control de la anticoagulación con las cumarinas, particularmente para la detección de interacciones desconocidas.
