OBJETIVO: Presentar una revisión abarcadora de la literatura acerca de medición, correlaciones y resultados de salud de la no-adhesión a tratamiento con medicamentos entre las personas de edad avanzada con domicilio comunitario.
FUENTES DE DATOS: Se llevó a cabo una búsqueda en las bases de datos MEDLINE, Pubmen, e International Pharmaceutical Abstracts de la literatura en el idioma inglés publicada entre enero de 1966 y diciembre de 2002. Se utilizaron 1 o más de los términos a continuación: persona de edad avanzada, adhesión/no-adhesión, cumplimiento/nocumplimiento, medicamento/droga, metodología/medición, y hospitalización.
SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE DATOS: Se seleccionaron para revisar los estudios de adhesión a medicamentos por personas de edad avanzada con domicilio comunitario además de publicaciones relevantes extraidas de las listas de referencias de los artículos identificados en la búsqueda inicial en las bases de datos.
SÍNTESIS DE DATOS: La medición exacta de adhesión a medicamentos continúa siendo difícil a pesar de haber disponibles varios métodos para su evaluación. La evidencia disponible sugiere que la polifarmacia y la pobre relación entre el paciente y el proveedor de salud (incluido el uso de múltiples proveedores) puede que sean determinantes mayores de noadhesión entre las personas de edad avanzada. El impacto de la mayoría de los factores sociodemográficos es de poca importancia. Existe poco consenso con relación a otros determinantes de no-adhesión. Relativamente muy pocas investigaciones de calidad alta han examinado las asociaciones entre no-adhesión y los resultados de salud. Datos disponibles proveen algún apoyo a que los riesgos a la salud aumentan con la no-adhesión. Sin embargo, las intervenciones para mejorar la adhesión a medicamentos pocas veces han demostrado efectos positivos en los resultados de salud.
CONCLUSIONES: Existen pocos datos empíricos para apoyar un descriptor sistemático simple de un paciente con no-adhesión a medicamentos. Las inconsistencias que presentan los estudios pueden atribuirse, en parte, a las dificultades inherentes a la medición de un factor de riesgo de la conducta como el de no-adhesión. Las investigaciones futuras en esta área se fortalecerían con la incorporación de evaluaciones detalladas de las razones que informan los pacientes para la no-adhesión a medicamentos, cuan apropiados son los regímenes de medicamentos, y el efecto de la no-adhesión en los resultados de salud.
