OBJETIVO: Describir el primer caso de queratitis postraumática por Scedosporium apiospermum (ScA) tratada con éxito con voriconazol sistémico y tópico.

RESUMEN DEL CASO: Varón de 19 años que acude a nuestro hospital con herida incisa en ojo izquierdo y córnea totalmente seccionada. El accidente se produjo con un cúter que era utilizado habitualmente en labores de jardinería. Tras el tratamiento empírico inicial, el paciente recibió voriconazol sistémico y tópico lo que evitó la enucleación del ojo. Transcurridos aproximadamente 5 meses desde el accidente, se practicó queratoplastia, extracción de catarata postraumática, e implantación de lente intraocular con evolución clínica favorable.

DISCUSIÓN: A pesar de que la queratitis por ScA es poco frecuente, debe sospecharse siempre que exista historia de daño ocular con objetos sucios y contaminados. Entre los antifúngicos disponibles, voriconazol muestra ventajas importantes como son la disponibilidad de una formulación oral y menor CMI para este patógeno.

CONCLUSIONES: Voriconazol se presenta como una excelente alternativa en el tratamiento de la queratitis por ScA. Tiene la ventaja añadida de disponer de formulación oral lo cual es de vital importancia ante la larga duración de los tratamientos en este tipo de infecciones.
