DRUG INFORMATION ROUNDS

OBJETIVO: Evaluar la efectividad de acetilcisteina administrada por la vía intravenosa (IV) en el tratamiento de fallo hepático agudo que no es inducido por acetaminofén.

FUENTES DE DATOS: Se llevó a cabo una búsqueda en las bases de datos de MEDLINE (1966–marzo 2003), IPA (1970–2003), y Cochrane Library (2003, Publicación Número 3). Se utilizaron los términos acetilcisteina, fallo hepático no inducido por acetaminofén, fallo hepático, intravenoso y tratamiento.

SÍNTESIS DE DATOS: Todos los estudios clínicos identificados fueron con muestras de pacientes de tamaño pequeño y no proveen evidencia concluyente de que la N-acetilcisteina beneficia este subgrupo de pacientes. Se observaron beneficios microvasculares regionales pero no se estudiaron los resultados clínicos.

CONCLUSIONES: La acetilcisteina IV no debe ser utilizada como tratamiento inicial en fallo hepático agudo que no es inducido por acetaminofén. Para poder considerarse se necesitan estudios clínicos con muestras de tamaño grande donde se evalúen los resultados clínicos.
