AMBULATORY CARE

ANTECEDENTES: Las responsabilidades del farmacéutico en el cuidado de pacientes con diabetes mellitus (DM) se están ampliando. Aún así, hay muy pocos datos disponibles en apoyo de la optimización de la terapia y la mejora de los resultados en estos pacientes debido a la intervención del farmacéutico.

OBJETIVO: Determinar el efecto de una clínica para el manejo de diabetes dirigida por un farmacéutico clínico, sobre el control glicémico y los parámetros cardiovasculares y renales en pacientes con DM tipo 2.

MÉTODOS: Se condujo un estudio prospectivo y no aleatorio en 70 pacientes de la Administración de Asuntos de Veteranos. Los pacientes se reunieron con el farmacéutico cada 6 a 8 semanas por aproximadamente 30 minutos para recibir educación, asesoramiento sobre medicamentos, seguimiento, y manejo. La finalidad principal fue el impacto de la participación en la clínica por 9 a 12 meses sobre la HbA1c. También se evaluó el peso, el colesterol total, la lipoproteína colesterol de baja densidad (LDL-C, por sus siglas en inglés), la lipoproteína colesterol de densidad elevada (HDL-C, por sus siglas en inglés), los triglicéridos, la presión sanguínea sistólica y diastólica, y el nivel de microalbuminuria. Todas las comparaciones se hicieron usando una prueba t-pareada con un nivel de significación de p < 0.05.

RESULTADOS: La HbA1c disminuyó significativamente de un nivel base de 10.3% ± 2.2% a 6.9% ± 1.1% en el período de evaluación de 9–12 meses (p < 0.001). La presión sistólica (p < 0.001) y la diastólica (p < 0.001), el colesterol total (p < 0.001), LDL-C (p < 0.001), los triglicéridos (p = 0.006), y el nivel de microalbuminuria (p < 0.001) también fueron reducidos en el período de 9–12 meses.

CONCLUSIONES: Este estudio demostró que un farmacéutico clínico puede cuidar efectivamente a pacientes con DM que le son referidos por un proveedor de cuidado primario debido a pobre control glicémico.
