OBJETIVO: Informar el caso de un infarto agudo de la pared lateral del miocardio (IAM) el cual coincidió con el uso de un suplemento nutricional que contenía Citrus aurantium L. (naranja amarga; en inglés, Bitter Orange) en un paciente sin historial de enfermedad coronariana.

RESUMEN DEL CASO: Una mujer de raza caucásica de 55 años se presentó a la sala de emergencia con síntomas de dolor de pecho y en un hombro. Su historial reveló el consumo de un suplemento nutricional compuesto de varios ingredientes, incluyendo 300 mg de naranja amarga (Edita's Skinny Pill) durante el pasado año. Su historial médico indicaba que era fumadora, pero no padecía hipertensión, enfermedad coronariana, o hiperlipidemia. Un arteriograma reveló una lesión en la arteria coronaria principal izquierda. La paciente fue diagnosticada con infarto agudo de la pared lateral del miocardio y estuvo hospitalizada por 4 días.

DISCUSIÓN: Los consumidores consideran que los suplementos nutricionales son generalmente seguros. Sin embargo, algunos de los suplementos que se utilizan para perder peso tienen ingredientes que se han asociado a eventos cardiovasculares. Aunque los consumidores están tomando mayor conciencia de los efectos adversos de productos conteniendo ingredientes tales como Ma Huang y efedra, los informes involucrando otros ingredientes están en aumento. C. aurantium (naranja amarga) o sinefrina, el cual está presente en la naranja amarga, se ha asociado con reacciones cardiovasculares adversas. Basándose en la escala de probabilidades Naranjo, el C. aurantium está posiblemente asociado a este evento cardiovascular.

CONCLUSIONES: El uso de suplementos que contienen C. aurantium pueden representar un riesgo de toxicidad cardiovascular, pero se necesitan estudios e informes adicionales para validar esta conclusión.
