OBJETIVO: Informar sobre 2 casos de ataxia cerebelosa transitoria relacionada con la administración tópica de lidocaína para endoscopia.

RESUMEN DEL CASO: Dos pacientes desarollaron ataxia cerebelosa transitoria pocos minutos después de recibir anestesia local con lidocaína antes de broncoscopia y ecocardiographía transesofágico. El efecto desapareció completamente dentro de 3–5 horas. No se pudo identificar ninguna otra etiología para la ataxia cerebelosa en estos 2 casos. Uno de los pacientes había sufrido un reacción similar a la lidocaína previamente.

DISCUSIÓN: Se han reportado varios efectos neurológicos centrales con la lidocaína, pero solamente pocos casos de ataxia cerebelosa. Según la Escala Naranjo de probabilidad de reacciones adversas, la relación entre la administración de la lidocaína y la ataxia cerebelar transitoria se considera probable o altamente probable.

CONCLUSIONES: Se puede observar la ataxia cerebelosa después de usar lidocaína para anestesia local. Así, debe tomarse cuidado para no sobredosificar la lidocaína, aún cuando se administra tópicamente.
