DRUG INFORMATION ROUNDS

OBJETIVO: Llevar a cabo una revisión acerca de la evidencia de la utilidad de las estatinas en la prevención secundaria de la enfermedad coronariana en pacientes con niveles de colesterol de lipoproteínas de baja densidad (LDL-C por sus siglas en inglés) óptimos o casi óptimos.

FUENTES DE INFORMACIÓN: Se utilizó el sistema de información MEDLINE (1966 a octubre 2003) usando los términos inhibidores de HMG-CoA reductasa, estatinas, enfermedad coronariana, post infarto de miocardio, y colesterol promedio.

SÍNTESIS: Se revisaron estudios de prevención secundaria que utilizaron sujetos con niveles de línea base de LDL-C casi óptimos (<130 mg/dL) u óptimos (<100 mg/dL). Los estudios de prevención secundaria utilizando estatinas que se llevaron a cabo al inicio sugirieron beneficios atenuados. Los estudios más recientes cuestionan estos hallazgos.

CONCLUSIONES: La prevención secundaria de la enfermedad coronariana utilizando las estatinas en aquellos pacientes con LDL-C cercanos a la meta establecida es controversial, aunque la evidencia de los estudios recientes parece ser prometedora.
