PEDIATRICS

OBJETIVO: Revisar la indicación, la farmacología, la farmacocinética, la eficacia, y los efectos adversos de la hormona de crecimiento humana recombinada en niños con baja estatura idiopática.

FUENTES DE INFORMACIÓN: Se llevaron a cabo búsquedas en MEDLINE y PubMed desde 1966 hasta diciembre 2003 con los siguientes términos de búsqueda: hormona de crecimiento humana, somatropina, Humatrope, niños normales, somatrem, y baja estatura idiopática. Además, se revisó el acta de la reunión del Comité Asesor de la FDA.

SELECCIÓN DE FUENTES DE INFORMACIÓN Y MÉTODO DE OBTENCIÓN DE LA INFORMACIÓN: La información presentada en esta revisión se obtuvo a partir de literatura científica publicada, de resúmenes presentados en reuniones científicas, y de información de archivos del fabricante. También, se identificaron artículos de fuentes adicionales. Se revisaron revistas actualizadas de pediatría y endocrinología para obtener los artículos más recientes. Sólo se utilizaron los artículos que trataban el tema del uso de la hormona de crecimiento en niños normales y sanos.

SÍNTESIS: La somatropina está indicada en niños con baja estatura idiopática. Algunos estudios han demostrado un pequeño beneficio sobre la estatura final alcanzada; en las dosis utilizadas para tratar la baja estatura idiopática, no ha habido efectos adversos relacionados con somatropina.

CONCLUSIONES: Se ha encontrado que la hormona de crecimiento mejora levemente la estatura final de niños con baja estatura idiopática. Se desconoce la población concreta de pacientes con mayor probabilidad de lograr el beneficio máximo, las pautas de dosificación óptimas, ni los efectos adversos a largo plazo del tratamiento crónico.
