TRANSPLANTATION

OBJETIVO: Revisar la literatura relacionada a la medición de fármacos inmunosupresores según se lleva a cabo en laboratorios de seguimiento farmacoterapéutico asociados con centros de transplante. Considerar si un método de prueba comúnmente usado para manejo de dosis de pacientes alcanza los criterios de calidad aceptable en el contexto de otras fuentes de variabilidad con estos fármacos.

FUENTE DE DATOS: Los artículos usados fueron obtenidos principalmente a través de MEDLINE y referencias citadas en publicaciones relacionadas. La búsqueda fue restringida a transplante de órganos en humanos.

SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE DATOS: El énfasis fue en la literatura relacionada a la calidad de las pruebas para fármacos inmunosupresores, sus limitaciones, y en la evidencia del beneficio clínico de individualizar la dosis.

SÍNTESIS DE DATOS: Existe un dilema evidente entre la calidad de los servicios analíticos ofrecidos por algunos fabricantes de pruebas diagnósticas y la capacidad de un número significativo de laboratorios clínicos globales para seleccionar sólo los métodos de prueba apropiados.

CONCLUSIONES: En muchos casos, los laboratorios clínicos fallan en alcanzar expectativas clínicas razonables requeridas para la interpretación de los resultados de las pruebas de fármacos inmunosupresores como un apéndice a la individualización óptima de la dosis y al cuidado al paciente.
