AMBULATORY CARE

OBJETIVO: Revisar la eficacia de los macrólidos y las teraciclina en el tratamiento de varias enfermedades inflamatorias crónicas.

FUENTES DE INFORMACIÓN: Se identificaron artículos sobre el tema mediante una búsqueda en el sistema de información MEDLINE (1966-marzo 2004) así como una búsqueda bibliográfica extensa. Las palabras claves usadas para la búsqueda incluyeron acné, blefaritis, enfermedad cardiovascular, fibrosis quística, periodontitis, rosácea, y artritis reumatoidea.

SÍNTESIS: Frecuentemente se prescriben tratamientos prolongados de antibióticos durante meses o incluso años para enfermedades como acné, blefaritis, enfermedad cardiovascular, fibrosis quística, periodontitis, rosácea, y artritis reumatoidea. Ensayos clínicos controlados y con asignación aleatoria han demostrado que la azitromicina es útil en el manejo de fibrosis quística y que las tetraciclinas son beneficiosas en el tratamiento de acné, blefaritis, periodontitis, y artritis reumatoidea. Algunos ensayos clínicos controlados con asignación aleatoria no han conseguido demostrar un beneficio de los macrólidos en la prevención secundaria de enfermedad cardiovascular. No se encontraron estudios controlados sobre la eficacia de los macrólidos o tetraciclinas en pacientes de rosácea.

CONCLUSIONES: El uso de macrólidos o tetraciclinas para rosácea para se basa principalmente en informes anecdóticos o en estudios no controlados. Algunos estudios limitados apoyan el uso de macrólidos o tetraciclinas en acné, blefaritis, periodontitis, artritis reumatoidea, y fibrosis quística. Hasta el momento, los estudios no apoyan la utilización de antibióticos en la prevención primaria de la enfermedad cardiovascular.
