PEDIATRICS

OBJETIVO: Repasar las guiás actualizadas del tratamiento de otitis media en pacientes pediátricos proveídos por la American Academy of Pediatrics (AAP) y American Academy of Family Physicians (AAFP).

FUENTES DE INFORMACIÓN: Se realizó una búsqueda en MEDLINE limitado al idioma inglés en pacientes pediátricos y usando los términos acute otitis media, guideline, observation therapy, y vaccination. Referencias adicionales fueron localizadas por medio de una revisión de las bibliografías de los artículos citados.

SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE DATOS: Se incluyó estudios que estaban relacionadas a la base fundamental de las guiás actualizadas y artículos que trataban los temas actuales de infecciones de otitis media.

SÍNTESIS DE DATOS: La otitis media afecta a muchos pacientes pediátricos en los Estados Unidos. Se ha generado preocupación sobre el tratamiento apropiado de la otitis media aguda como resultado de un aumento en la resistencia entre los patógenos responsables a esta infección. Algunos países han elegido observar los pacientes por un periódo de tiempo señalado antes de la iniciación de tratamiento con antibióticos. La AAP y AAFP han puesto al día las guías para el tratamiento de otitis media y han incluido terapia de observación como una opción, recomendaciones para la dosificación de varios regimientos de antibióticos y sus lugares en el tratamiento, y la importancia del manejo inicial de dolor.

CONCLUSIONES: Las guiás actualizadas para el tratamiento de otitis media han sido desarrollado con la intención de tratar correctamente a los niños mientras disminuyendo el porcentaje de resistencia actual en los organismos implicados en el desarrollo de la otitis media aguda. En el futuro, los resultados de la terapia de observación relacionadas con la incidencia de resistencia a antibióticos y la posibilidad de complicaciones crecientes relacionados con las complicaciones de otitis media deberán ser evaluados en los Estados Unidos.
