OBJETIVO: Notificar 5 casos de una interacción de warfarina con moxifloxacina que resultaron en una elevación de la razón normalizada internacional (RNI) en todos ellos y hemorragia clínicamente significativa en un caso.

RESUMEN DE LOS CASOS: Entre enero de 2002 y enero de 2004, se identificaron retrospectivamente los casos de 4 hombres y una mujer con edades comprendidas entre 63 y 92 años, que habían presentado elevación significativa de la RNI tras haberles prescrito moxifloxacina concomitantemente con warfarina.

DISCUSIÓN: Esta es la segunda serie de casos notificada que describe una interacción de warfarina y moxifloxacina. La actual monografía de producto de moxifloxacina indica que este fármaco no tiene efecto significativo en la farmacocinética de warfarina-R o -S o en el tiempo de protrombina (RNI). Una interacción moxifloxacina–warfarina probablemente produjo hospitalización prolongada en 2 casos y hemorragia gastrointestinal significativa en un caso. En 3 de los 5 casos, la interacción moxifloxacina–warfarina se calificó como probable a partir del algoritmo de probabilidad de Naranjo. En los otros 2 casos restantes, la interacción se calificó de posible.

CONCLUSIONES: Los autores concluyen que los profesionales de la salud deben considerar el potencial de interacción de moxifloxacina con warfarina. La monitorización frecuente y de rutina de la RNI en pacientes tratados con warfarina y previamente estabilizados, al iniciar o abandonar moxifloxacina, puede ayudar a detectar esta interacción potencial.
