NEW DRUG APPROVALS

OBJETIVO: Revisar pemetrexed, un novedoso agente antifolato multidiana.

FUENTES DE INFORMACIÓN: La búsqueda bibliográfica (1985–septiembre 2004) se realizó a partir de la base de datos MEDLINE y CANCERLIT. También se incluyeron extractos recientes procedentes de la American Society of Clinical Oncology (ASCO) junto con la información del fabricante. Las palabras clave fueron pemetrexed, LY-231514, Alimta, antifolato multidiana, y mesotelioma pleural maligno.

SELECCIÓN DE FUENTES DE INFORMACIÓN Y MÉTODO DE OBTENCIÓN DE INFORMACIÓN: Se seleccionó información relevante sobre la farmacología, farmacocinética, y seguridad y eficacia de pemetrexed en estudios clínicos.

SÍNTESIS: Pemetrexed inhibe el metabolismo del folato y la síntesis de purina-pirimidina. En base a los estudios clínicos de fase I/II, pemetrexed tiene una actividad antitumoral en tumores sólidos como pulmón, colorectal, y cervical. Un estudio de fase III en pacientes con mesotelioma pleural maligno (MPM) demostró una supervivencia superior con la terapia conjunta con pemetrexed y cisplatino en comparación con la terapia única con cisplatino.

CONCLUSIONES: Pemetrexed es un nuevo fármaco prometedor para el tratamiento de neoplasias sólidas, especialmente MPM.
