OBJETIVO: La enfermedad de Still de inicio en la edad adulta es una enfermedad rara con manifestaciones clínicas heterogéneas. Aunque se han utilizado varios fármacos para su tratamiento, no hay estudios controlados de las diferentes modalidades de tratamiento. Se describe el caso de un paciente con enfermedad de Still de inicio en la edad adulta tratado con éxito con leflunomida y azatioprina.

RESUMEN: Una mujer de 24 años con enfermedad de Still de inicio en la edad adulta fue tratada inicialmente con indometacina, corticosteroides, e hidroxicloroquina. Al no haber mejoría clínica, a su pauta se añadió metotrexate y, luego, azatioprina. Como la paciente todavía sufría exacerbaciones con este tratamiento, se inició ciclofosfamida tras dretirar la azatioprina y el metotrexato. Sin embargo, debido a la actividad persistente de la enfermedad, se administró leflunomida combinada con azatioprina. Solamente con esta pauta se pudo lograr el control completo de la enfermedad con una normalización de la velocidad de sedimentación de eritrocitos, proteína C-reactiva, y niveles de ferritina. Durante el seguimiento de un año, no se observaron recurrencias ni efectos adversos atribuibles a leflunomida o azatioprina.

DISCUSIÓN: La experiencia clínica del uso combinado de leflunomida y azatioprina en la enfermedad de Still de inicio en la edad adulta es limitada. Esta combinación puede estar modificando la expresión clínica de esta enfermedad a través de sus efectos en la expansión clonal de los linfocitos T y la producción de citoquinas proinflamatorias.

CONCLUSIONES: La combinación de leflunomida y azatioprina parece ser eficaz y segura en el tratamiento de enfermedad de Still de inicio en la edad adulta.
