PSYCHIATRY

INTRODUCCIÓN: Debido a la alta prevalencia de depresión en mujeres en edad fértil y con pareja, y al hecho de que el 50% de los embarazos no son planificados, existe una alta probabilidad de que estas mujeres hayan sido expuestas al uso de antidepresivos durante las primeras fases del embarazo.

OBJETIVO: Determinar las tasas basales de aborto espontáneo (AE) y si los antidepresivos aumentan estos niveles.

MÉTODOS: Los niveles de AE en las mujeres que tomaban antidepresivos comparados con los de las mujeres sin depresión se combinaron en un riesgo relativo utilizando los modelos de efectos aleatorios. Para la identificación de los estudios de cohortes se utilizaron las bases de datos MEDLINE, EMBASE, Healthstar, Toxline, Psychlit, Cochrane database, and Reprotox publicados durante el período 1966–2003.

RESULTADOS: De los 15 artículos potenciales, 6 estudios de cohortes de 3567 mujeres (1534 expuestas, 2033 no expuestas) proporcionaron datos para el análisis. Todos ellos fueron apareados por variables de confusión importantes. Las pruebas no encontraron heterogeneidad ({chi}2 3.13; p = 0.98); todas las puntuaciones de calidad fueron adecuadas (>50%). La tasa basal de AE (CI 95%) fue del 8.7% (7.5%–9.9%; n = 2033); para las mujeres en tratamiento con antidepresivos, el nivel fue de 12.4% (10.8%–14.1%; n = 1534), con un incremento significativo del 3.9% (1.9%–6.0%); RR 1.45 (1.19–1.77; n = 3567). No se encontraron diferencias entre los distintos tipos de antidepresivos.

CONCLUSIONES: La exposición materna a los antidepresivos podría estar asociada a un incremento del riesgo de AE, aunque la depresión por si misma no puede ser descartada.
