NEW DRUG DEVELOPMENTS

OBJETIVO: Etoricoxib es un inhibidor COX-2 altamente selectivo que no está aprobado actualmente para uso en EUA. La literatura disponible que evalúa la farmacología, la farmacocinética, la eficacia clínica, y los efectos adversos será repasada.

FUENTES DE DATOS: La literatura fue obtenida a través de MEDLINE, Current Contents, y la biblioteca Cochrane. Las referencias de los artículos encontrados, información del fabricante, y extractos de las convenciones del Colegio Americano de Reumatología y el Congreso Anual Europeo de Reumatología fueron repasados.

SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE INFORMACIÓN: Todos los estudios clínicos publicados evaluando etoricoxib fueron incluidos en este repaso. Un extracto era excluido si presentaba datos preliminares de estudios que estaban publicados, datos analizados reportados previamente en un estudio clínico publicado, o si comparaba etoricoxib con placebo para una indicación con estudios clínicos publicados comparando con otro activo.

SÍNTESIS: Doce estudios clínicos evaluando eficacia para dolor agudo y crónico fueron repasados. Para seguridad, se repasaron 3 estudios clínicos y 6 análisis retrospectivos.

CONCLUSIONES: Los estudios disponibles demuestran eficacia de etoricoxib comparado con NSAIDs, pero ningún estudio publicado hasta la fecha compara etoricoxib con otros inhibidores selectivos COX-2. Mientras estos agentes han demostrado una reducción significativa en efectos adversos GI, los efectos adversos cardiovasculares de la inhibición selectiva de COX-2 no están bien definidos. Más estudios son necesarios para delinear los beneficios y riesgos de etoricoxib comparado con regimenes alternativos de tratamiento.
