DRUG INFORMATION ROUNDS

OBJETIVO: Evaluar el papel del óxido nítrico inhalado (ONi) en receptores de trasplantes de corazón o pulmón.

FUENTES DE INFORMACIÓN: Literatura científica identificada por medio de una búsqueda en MEDLINE (de 1966 a julio de 2004).

SÍNTESIS: La hipertensión pulmonar causada por insuficiencia ventricular derecha o por lesiones de reperfusión isquémica son, respectivamente, complicaciones que surgen después de un trasplante de corazón o de pulmón. Esta revisión de 7 estudios publicados analiza el papel del ONi en estas poblaciones y ofrece recomendaciones de tratamiento, dosificación y monitorización basadas en pruebas.

CONCLUSIONES: El ONi muestra beneficios hemodinámicos en pacientes con trasplante de corazón recién operados que padecen hipertensión pulmonar preexistente; también proporciona beneficios hemodinámicos variables en receptores de trasplante de pulmón. Actualmente, los datos de morbilidad y mortalidad no favorecen ninguna de las dos indicaciones. Su uso es meramente de apoyo y requiere estudios adicionales.
