GASTROENTEROLOGY

OBJETIVO: Revisar los adelantos recientes que han cambiado significativamente el manejo de la infección crónica del virus de hepatitis C (HCV, por sus siglas en inglés).

FUENTES DE INFORMACIÓN: Se realizó una búsqueda de la literatura en el sistema MEDLINE (enero 2000–junio 2005) utilizando palabras claves en inglés como hepatitis C, interferon, pegylated interferon, y therapy.

SELECCIÓN DE FUENTES INFORMACIÓN Y MÉTODO DE EXTRACCIÓN DE INFORMACIÓN: Se identificaron todos los artículos correspondientes al tratamiento de la infección crónica de HCV. Los estudios evaluando el tratamiento de HCV en pacientes que no habían sido expuestos previamente a tratamiento fueron considerados para esta revisión.

SÍNTESIS: En los últimos años, la respuesta al tratamiento de la infección crónica de HCV ha mejorado significativamente con el uso de interferón pegilado y la terapia con ribavirin. La respuesta al tratamiento es afectada por el genotipo de HCV, la carga viral, al igual que por factores relacionados con el paciente, incluyendo adherencia al tratamiento.

CONCLUSIONES: El tratamiento de la infección crónica de HCV ha mejorado, con una razón de respuesta completa de aproximadamente 55%. La identificación y el manejo de los efectos adversos comunes es importante para maximizar la adherencia y la respuesta a la terapia. Se necesitan estudios para delinear más ampliamente el tratamiento óptimo de la infección crónica de HCV en poblaciones específicas de pacientes.
