DRUG INFORMATION ROUNDS

OBJETIVO: Evaluar el uso del azul de metileno para el tratamiento de encefalopatía inducida por la ifosfamida.

FUENTES DE INFORMACIÓN: Se llevó a cabo una búsqueda en MEDLINE (1966–agosto 2005) e International Pharmaceutical Abstracts (1971–agosto 2005) utilizando los términos de búsqueda: azul de metileno, ifosfamida, encefalopatía, y neurotoxicidad.

SÍNTESIS: Varios informes de casos y una revisión retrospectiva de expedientes describieron el uso del azul de metileno para la encefalopatía inducida por la ifosfamida, pero no se encontraron estudios clínicos. En algunos pacientes, el azul de metileno parece ayudar en la eliminación de los síntomas de la encefalopatía tan pronto como en 10 minutos después de su administración; sin embargo, su eficacia fue moderada en la mayoría de los pacientes. En otros pacientes, los síntomas desaparecieron en el mismo tiempo en el cual desaparecen los síntomas de encefalopatía inducida por ifosfamida sin recibir ningún tratamiento.

CONCLUSIONES: La información disponible de informes de casos indica que el azul de metileno es una opción en el tratamiento de la encefalopatía inducida por la ifosfamida, especialmente en pacientes con síntomas de toxicidad severa. Sin embargo, la falta de estudios clínicos controlados y la posibilidad de que la encefalopatía se resuelva espontáneamente no muestran claramente la utilidad del azul de metileno.
