ANTICOAGULATION

OBJETIVO: Revisar el riesgo de tromboembolismo venoso (TEV) en varias poblaciones de pacientes y evaluar los agentes disponibles para la prevención y tratamiento de TEV utilizando un estilo basado en casos clínicos.

FUENTES DE INFORMACIÓN: Se realizó una búsqueda en la Biblioteca Nacional de Medicina (1995-julio 2006) para identificar la literatura relevante. Se revisaron referencias adicionales de los artículos seleccionados.

SELECCIÓN DE FUENTES DE INFORMACIÓN: Se revisaron artículos relacionados a la prevención de TEV en cirugía ortopédica, cirugía general, y en pacientes médicamente enfermos, así como al tratamiento de TEV.

SÍNTESIS: Las opciones farmacológicas para la prevención y tratamiento de TEV incluyen warfarina, heparina no-fraccionada (HNF), heparinas de bajo peso molecular (HBPM), y fondaparinux. Las guías vigentes apoyan el uso de warfarina, HBPM o fondaparinux para profilaxis de TEV luego de cirugía ortopédica mayor de las extremidades inferiores. Para profilaxis de TEV en pacientes hospitalizados por condiciones médicas o pacientes de cirugía general, se recomienda usar HNF y HBPM; sin embargo estudios recientes sugieren que fondaparinux es también efectiva en este grupo de pacientes. El uso de HNF o de HBPM (ambas en conjunto con warfarina) es recomendado en el tratamiento de TEV o embolia pulmonar no-invasiva. Información reciente sugiere que fondaparinux (en conjunto con warfarina) también es efectiva en el tratamiento de TEV. Una variedad de factores farmacocinéticos, farmacodinámicos, y farmacoeconómicos diferencian estos agentes para las varias indicaciones.

CONCLUSIONES: Este repaso de las opciones farmacológicas disponibles para la prevención y el tratamiento de TEV resalta la necesidad de la disponibilidad de una variedad de agentes para manejar esta condición. Al momento no existe un sólo anticoagulante que cubra todas las necesidades. Una variedad de factores de pacientes, incluyendo el tipo de cirugía, indicación médica, factores de riesgo para tromboembolismo, riesgo de sangrado, historial de trombocitopenia inducida por heparina, y una gran variedad de condiciones comórbidas pueden afectar la seguridad, eficacia y selección apropiada de la terapia para TEV.
