Potenciación del Efecto de Warfarina por Chitosan

OBJETIVO: Informar un caso donde el efecto anticoagulante de warfarina aparenta haber sido potenciado por chitosan, probablemente debido a interferencia en la absorción de vitamina K.

RESUMEN DEL CASO: Un hombre de 83 años de edad con enfermedad cardiovascular hipertensiva, diabetes mellitus tipo 2, y fibrilación atrial crónica, complicada por la formación de un trombo en el atrio izquierdo, recibía terapia de mantenimiento con warfarina 2.5 mg/día. Una elevación marcada en la razón normalizada internacional (INR) se observó, luego de que se automedicara con chitosan 1200 mg 2 veces al día. El niega haber estado tomando otros medicamentos, sustancias naturales, medicinas herbarias y suplementos nutricionales, tampoco él cambió el hábito alimenticio. Luego que se le administró vitamina K parenteral y se le descontinuó el chitosan, el INR regresó a su rango terapéutico. Bajo fuertes recomendaciones médicas, él dejó de tomar el chitosan y el INR se mantuvo estable de ahí en adelante.

DISCUSIÓN: Chitosan es un polímero cargado positivamente el cual se enlaza a los lípidos cargados negativamente y a los ácidos biliares del tracto gastrointestinal. Puede afectar la absorción de las vitaminas A, D, E, y K. Por lo tanto, el efecto anticoagulante de warfarina puede ser potenciado por chitosan a través de este mecanismo. Una evaluación objetiva de la causalidad (Escala de Probabilidad de Reacciones Adversas a Drogas de Naranjo) también revela que el efecto adverso a la droga fue probable.

CONCLUSIONS: La interacción entre warfarina y chitosan no se ha reportado previamente. Los profesionales del cuidado de la salud deben estar conscientes de esta interacción potencial.
