ANTICOAGULATION

OBJETIVO: Revisar la literatura existente sobre la incidencia de trombosis en pacientes oncológicos con catéteres venosos centrales, y sopesar la evidencia que apoya la tromboprofilaxis en esta población de pacientes.

FUENTES DE INFORMACIÓN: La búsqueda bibliográfica se realizó a partir de la base de datos MEDLINE (1966—febrero 2007) mediante los siguientes descriptores: tumor, cáncer, catéter, profilaxis, trombosis, catéter venoso central.

SÍNTESIS: Se han observado varias variables entre los ensayos clínicos publicados, lo que dificulta las comparaciones para determinar la presencia de beneficios clínicos. Generalmente, el uso de warfarina a baja dosis, heparina de bajo peso molecular o heparina no fraccionada a baja dosis no alcanzó significancia estadística consistente en la reducción de trombosis por catéter venoso central (CVC).

CONCLUSIONES: Los datos disponibles no apoyan el uso rutinario de anticoagulantes como tromboprofilaxis para la prevención de la trombosis por CVC. Sin embargo, se han observado varias inconsistencias en los estudios llevados a cabo hasta la fecha. Se necesitan más estudios para identificar subconjuntos de pacientes oncológicos que tienen un mayor riesgo de desarrollar trombosis por CVC y que pueden beneficiarse de una anticoagulación sistémica para la prevención de la trombosis por CVC.
