Actualización de Abatacept: Un Modulador Selectivo de la Co-Estimulación para Artritis Reumatoide

SELECCIÓN DE ESTUDIOS: Todos los estudios en animales y en humanos describiendo la farmacología de abatacept y los estudios en humanos describiendo la farmacocinética, farmacodinámica, eficacia, seguridad, eventos adversos, y las precauciones para abatacept fueron incluidos.

SÍNTESIS DE DATOS: Abatacept mejora significativamente los signos y síntomas de artritis reumatoide moderada a severa en pacientes que han experimentado una respuesta inadecuada a metotrexato o a inhibidores del factor-{alpha} de necrosis de tumor. Aproximadamente 50% de los pacientes alcanzaron remisión (definido como puntación de actividad de enfermedad [DAS-28] de menos de 2.6) para el mes 12 que se mantuvo por lo menos por 24 meses de terapia. Los eventos adversos más comunes incluyen dolor de cabeza, infecciones del trayecto respiratorio superior, nauseas, y nasofaringitis. Eventos adversos raros pero serios incluyen infecciones serias y malignidad.

CONCLUSIONES: Abatacept ha documentado eficacia y seguridad en pacientes con respuestas inadecuadas a metotrexato y agentes de factor contra necrosis de tumor en estudios de corto y largo plazo.
