Interacciones Clínicamente Significativas Fármaco-Fármaco entre Agentes Orales Anti-cáncer y Otros Agentes no Oncológicos: Descripción y Comparación de dos Compendios Farmacéuticos

TRASFONDO: Agentes orales oncológicos están ganando amplia aceptación para el tratamiento de cáncer, sin embargo los pacientes recibiendo dichas terapias están en alto riesgo de interacciones fármaco-fármaco (IFF). Con el propósito de aumentar la conciencia sobre las IFF entre los agentes orales oncológicos, este estudio pretende identificar las IFF relevantes y evaluar las inconsistencias entre 2 de los compendios farmacéuticos más comúnmente usados.

OBJETIVO: Crear un perfil de medicamento para cada IFF clínicamente significativa involucrando agentes orales oncológicos y evaluar las concordancias entre dos compendios comúnmente usados: Drug Interaction Facts (DIF) 2008 y Micromedex DRUGDEX.

MÉTODOS: Se desarrollaron perfiles de IFF basado en la revisión de la literatura primaria y terciaria. Se compararon DIF 2008 and Micromedex DRUGDEX para evaluar la consistencia de los listados, la severidad, y evidencia científica de las clasificaciones de los IFF de los agentes orales anticáncer seleccionados. Se utilizó la prueba de correlación de Spearman para comparar la severidad entre los 2 compendios.

RESULTADOS: Se identificaron un total de 184 IFF. Se creó un perfil para 40 IFF que reunieron los criterios predeterminados para los IFF. La evaluación comparativa demostró inconsistencia en los listados de IFF (15.2% de los IFF identificados estaban listados en el DIF solamente y 46.7% estaban en el listado de Micromedex solamente), la clasificación de severidad (coeficiente de correlación Spearman = 0.49), y clasificación de evidencie científica (desacuerdo = 25.8%).

CONCLUSIONES: Se identificaron IFF de importancia clínica para los pacientes de cáncer tomando agentes orales anticáncer. Las discrepancias en IFF entre las listas y los sistemas de clasificación entre los compendios reflejan la necesidad de más estudios para estandarizar las definiciones y clasificaciones de los IFF.
