Opciones de Tratamiento en la Leucemia Mielógena Crónica Resistente a Imatinib

OBJETIVO: Discutir las nuevas opciones terapéuticas disponibles para el tratamiento de pacientes con leucemia mielógena crónica (LMC) que no han tolerado la terapia con imatinib o en los que ésta no ha resultado efectiva.

FUENTES DE INFORMACIÓN: Se realizó una búsqueda bibliográfica a través de las bases de datos de MEDLINE (1966-mayo 2007), EMBASE (1991-3er trimestre 2007), las actas de la Sociedad Americana de Hematología (2000-2006), y las actas de la Sociedad Americana de Oncología Clínica (2000-2007). Entre los términos de búsqueda (en inglés) se incluyeron: imatinib, nilotinib, y leucemia mielógena crónica.

SELECCIÓN DE FUENTES DE INFORMACIÓN Y MÉTODO DE EXTRACCIÓN DE INFORMACIÓN: Estudios: Se incluyeron los extractos de conferencias y los estudios que se identificaron en lengua inglesa que hacían referencia a ensayos clínicos preclínicos y de fase 1, 2, y 3.

SÍNTESIS: Imatinib constituye el estándar de tratamiento para la LCM; sin embargo, algunos pacientes desarrollan resistencia o intolerancia al fármaco. Los datos clínicos procedentes de ensayos de fase I y II sobre dasatinib y nilotinib, inhibidores más potentes de la tirosina quinasa, son prometedores. Se han notificado respuestas hematológicas y citogenéticas con ambos fármacos. No parece haber resistencia cruzada entre los fármacos, aunque ninguno de ellos resulta efectivo contra todas las mutaciones del marcador molecular distintivo, el cromosoma Philadelphia. También se van a examinar agentes novedosos para el tratamiento de estos pacientes, incluidos los inhibidores de la aurora quinasa y la farnesil transferasa, así como terapias combinadas.

CONCLUSIONES: Dasatinib y nilotinib son opciones de segunda línea para el tratamiento de pacientes con leucemia mielógena crónica con resistencia o intolerancia al imatinib. Los perfiles de toxicidad entre los agentes pueden diferir. En la actualidad se están llevando a cabo varios ensayos clínicos con estos y otros agentes.
