Uso de Agonistas de 5-HT1 Durante el Embarazo

OBJETIVO: Informar y evaluar la información disponible sobre el uso de agonistas 5-HT1 (triptanos) durante el embarazo.

FUENTES DE INFORMACIÓN: Se realizó una búsqueda de literatura utilizando PubMed, para identificar estudios realizados en humanos y publicados en el idioma inglés entre 1990-diciembre 2007. Los términos utilizados para la búsqueda fueron embarazo, migraña, y los nombres individuales de los triptanos. Además, se contactaron los manufactureros de los 7 triptanos disponibles para obtener información sobre la existencia de registros de embarazo de sus productos y la disponibilidad de reportes de dichos registros.

SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE INFORMACIÓN: Se incluyeron y se evaluaron de forma estricta todos los estudios retrospectivos y prospectivos que informaron sobre indicadores clínicos de embarazo luego del uso de un triptano. Además se incluyó información de los registros apoyados por las casas manufactureras.

SÍNTESIS: Es imperativo manejar la migraña en una paciente embarazada de una manera segura y efectiva. La información relacionada al uso de sumatriptan, y en menor grado naratriptan y rizatriptan, existe principalmente para la exposición durante el primer trimestre. Esta información demuestra que no hay diferencias en malformaciones congénitas o resultados negativos de embarazo cuando se comparan a la tasa esperada en la población general o a las tasas observadas en los sujetos del grupo control. Existe muy poca información en relación a la exposición durante el segundo o tercer trimestre de embarazo..

CONCLUSIONES: Sumatriptan aparenta ser una alternativa de tratamiento segura para mujeres embarazadas que experimenten migraña durante el primer trimestre de embarazo. Se requiere de más observación antes de poder recomendar el uso de sumatriptan durante los últimos trimestres de embarazo. Basada en la información disponible el uso de otros agentes en la categoría de triptanos durante el embarazo no puede ser recomendados en este momento.
