El uso de Tigeciclina en el Tratamiento de Infecciones por Acinetobacter: una Serie de Casos

TRASFONDO: Las infecciones por Acinetobacter resistentes a múltiples clases de antibióticos han adquirido una incidencia considerable en muchas instituciones. La tigeciclina posee actividad in vitro contra las especies del Acinetobacter y se ha sugerido como una opción terapéutica en estas infecciones.

OBJETIVO: Describir los resultados clínicos y microbiológicos de pacientes que recibieron tigeciclina en el tratamiento de infecciones causadas por especies del Acinetobacter en nuestra institución.

MÉTODOS: Esta es una revisión retrospectiva de los expedientes médicos de 29 pacientes que recibieron tigeciclina en el tratamiento de infecciones causadas por Acinetobacter. Los resultados considerados para eficacia fueron mejoría o curación clínica y curación microbiológica en pacientes evaluables.

RESULTADOS: Los pacientes recibieron tigeciclina por un promedio de 30 días durante una hospitalización promedio de 11 días. Las indicaciones comunes fueron neumonía (15 pacientes), bacteriemia (6 pacientes), e infección urinaria (3 pacientes). Se observaron resultados clínicos positivos (curación o mejoría clínica) en 8 de 29 pacientes (28%). De los 23 pacientes evaluables por medios microbiológicos, 11 mostraron curación por medio de cultivos (46%). En once pacientes se llevaron a cabo pruebas de susceptibilidad, y la media de la CMI fue 4 Ìg/mL (rango 3–8 µg/mL).

CONCLUSIONES: En esta serie de casos, la mayoría de los pacientes no demostraron resultados clínicos ni microbiológicos favorables con la terapia de la tigeciclina. En ningún paciente se aisló alguna cepa totalmente susceptible a la tigeciclina. Se necesitan datos de más estudios antes de poder recomendar la tigeciclina para el tratamiento de infecciones por Acinetobacter.
