Nomogramas de Dosificación: Silos en una pendiente

El desarrollo de guías o protocolos para el manejo de dosificación podría tener varios beneficios para la estandarización del cuidado al paciente. Frecuentemente los clínicos tienen que incorporar múltiples factores al individualizar las terapias. En algunos casos, estrategias estandarizadas y/o pre-establecidas podrían crear barreras para individualizar el cuidado, potencialmente categorizando el paciente en una categoría de dosificación (silos) que minimiza sus necesidades individuales. Se han explorado estrategias para manejar los inhibidores directos de trombina y se han propuesto estrategias de dosificación diferentes a las provistas en la información del producto. Se discuten las consideraciones a observar al individualizar el manejo de terapia de anticoagulación, incluyendo el uso de protocolos de dosificación y seguimiento estandarizados.
