Continuidad en la Atención de Salud y el Farmacéutico: Vamos a Mantenerlo Sencillo

Abstract

Los farmacéuticos constituyen una piedra angular para el manejo de alta calidad de los medicamentos. La calidad en la continuidad de la atención involucra el manejo de la terapia de medicamentos enfocada en el paciente, lo cual podría promoverse para 26 millones de beneficiarios de la Parte D de Medicare. Sin embargo, muchos pacientes no reciben suficiente instrucción sobre sus medicamentos de receta. Los aspectos enfocados en el paciente son importantes debido a la heterogeneidad en los tipos de paciente y en la severidad de las enfermedades, requisitos de tratamiento que varían, factores del estilo de vida y diferencias en el conocimiento en salud. Aunque es aparente que existen limitaciones de tiempo para todos los profesionales de la salud, una contribución sencilla pero decisiva de parte de los farmacéuticos a la continuidad en la atención es aumentar su énfasis en brindar instrucciones sobre el uso de medicamentos a los pacientes.
